Listed below are some useful forms if you are a volunteer at UPC.

• Volunteer Handbook - A must for anyone planning an event or leading a ministry at UPC.

• Check Request Form - To request a check to pay an outside party beforehand (non-benevolence). 

• Benevolence Assistance Check Request Form - To request a check to pay an outside party on behalf of a mercy ministry request.

• Reimbursement for Benevolence Expenses Form (for Deacons & Deacon Assistants) - To be reimbursed for authorized mercy ministry expenses.

• Expense Reimbursement Form - To be reimbursed for authorized ministry expenditures you have already paid.

• Timesheet Form - To track your hours for paid workers.

• UPC Event Request Form - To request use of any of the buildings on campus. This form must be turned in 60 days prior to your event date.
